Rehab Aide/Coordinator
Job Summary
This hybrid role supports both administrative coordination and hands-on patient care within our Physical Therapy, Occupational Therapy, and Speech Therapy departments. The Aide will assist in delivering restorative therapy programs and support essential administrative functions to ensure smooth and effective therapy service delivery.

Core Responsibilities
Duties include but are not limited to:
Administrative Support
•Coordinate therapy schedules and manage patient authorizations and intake procedures.
•Handle insurance verifications and pre-authorizations.
•Support telehealth services for PT/OT/ST therapy sessions.
•Maintain timely and accurate documentation in the EMR system.
•Order equipment and therapy supplies; assist with faxing and filing therapy notes.
•Manage clinic supplies: restock linens and maintain cleanliness of the therapy gym and equipment.
Restorative Therapy Aide Duties
•Carry out therapist-directed restorative care plans for inpatients and swingbed patients.
•Provide hands-on support with therapeutic exercises, ambulation, ADL training, and assistive devices.
•Safely transport patients to and from therapy sessions.
•Document patient progress per patients care plans and report changes to the therapy team.
•Support swingbed and inpatient residents with restorative nursing programs.
•Assist with telehealth evaluations and treatments under licensed therapist guidance.
•Clean and prepare therapy areas and equipment between sessions.

Qualifications & Requirements
•High school diploma or equivalent.
•Minimum of 1 year experience in a healthcare or rehabilitation setting.
•Strong interpersonal and communication skills; team-oriented mindset.
•Proficiency in EMR systems and clinical documentation.
•Ability to complete work within designated timelines.
•Professional and positive representation of CCMH.
Preferred
•Certified Nursing Assistant (CNA) certification or previous Restorative Aide experience.
•Prior experience in a therapy or clinical administrative role.
•Familiarity with the Meditech system.

Physical Requirements
•Ability to walk, stand, bend, kneel, stoop, push/pull, and lift up to 40 pounds unassisted.
•Must possess good vision, hearing, verbal communication, and manual dexterity.
•Capable of safely assisting patients with transfers and mobility.
•Exposure to bodily fluids, communicable diseases, chemicals, and repetitive motion is possible.
•Must comply with all CMMH vaccination policies and safety protocols.
