Patient Rights & Responsibilities


CCMH recognizes that our patients have the right:
To be free from abuse, neglect or harassment;
To be treated with consideration and respect;
To protective oversight while a patient in the hospital;
To have personal possessions brought to the hospital reasonably protected;
To be informed of the hospital’s grievance policies/procedures, including who to contact and how;
To file a formal or informal verbal or written grievance and to expect a prompt resolution of the grievance, including a timely written notice of the resolution. The grievance may be made by the patient or the patient’s representative (A grievance is defined as any patient service or care issue that cannot be resolved promptly by staff present.);
To accept medical care or to refuse it to the extent permitted by law and to be informed of the medical consequences of refusal. The patient has a right appoint a surrogate to make health care decisions on his/her behalf to the extent permitted by law;
To be free from seclusion or restraints of any form that are not medically necessary.
To formulate advance directives and have hospital staff and practitioners comply with these directives.

CCMH recognizes that the patient or his/her designated representative have the right:
To be informed regarding the hospital’s plan of care for the patient;
To be informed, upon request, regarding general information pertaining to services received by the patient;
To review the patient’s medical record and to receive copies of the record at a reasonable photocopy fee;
To participate in the patient’s discharge planning, including being informed of service options that are available to the patient and a choice of agencies which provide the service;
To participate in treatment decisions and the care planning process.

CCMH asks that its patients take responsibility:
To provide accurate and complete information about present complaints, past illnesses, hospitalizations, medications and other matters relating to their health;
For following the treatment plan recommended by their physician;
For their actions/outcomes if they refuse or do not follow the established treatment plan;
For assuring their financial obligations for care are met;
For following all rules and regulations affecting patient care; and
For being considerate of the rights of other patients and hospital personnel.

Advance Directives

Patient's Rights to Self-Determination in Healthcare Decisions
Understandably, patients are often worried about the medical care they would receive if they became terminally ill and/or unable to communicate their wishes. Patients have the right to accept or refuse medical care and can protect the right to make medical decisions by putting their healthcare preferences in writing. This is called an advance directive. Different types of advance directives include a living will, a durable power of attorney for healthcare or any written document that is executed, dated and signed that expresses your healthcare treatment decisions.
If you have an advance directive, please inform our staff so that it can be noted and a copy placed in your medical record. You should keep the original advance directive, and bring it with you each time you are admitted to the hospital. If there are statements in your advance directive that cannot be honored at CCMH, they will be discussed with you and noted in your medical record. If you do not have an advance directive and would like to create one, or if you wish to obtain additional information, please contact Social Services at 417-876-3598 from an outside line.

Organ Donation

Federal regulations implemented in August 1998, require that hospitals notify an organ procurement organization (OPO) of all deaths that occur in the hospital. Because organ and tissue donation is an opportunity to make a positive and valuable contribution to another human being, our hospital will work with the OPO to ensure that the family of every potential donor knows about the option to donate organs or tissues.
