Financial Assistance

Cedar County Memorial Hospital is committed to providing quality health care to all patients. As a charitable, non-profit institution exempt from taxation under Section 501 (c)(3) of the Internal Revenue code, CCMH cares for the medically indigent residents of Cedar County by waiving fees for medically necessary services based upon the patient’s ability to pay, subject to the economic resources and wherewithal of CCMH.

To be eligible for a 100% reduction from charges (i.e., full write off) the patient’s household income must be at or below 200% of the current Federal Poverty Guidelines.

You may also call 417-876-2511 and request a policy and application be mailed to you. Applications may also be obtained at the CCMH Medical Mall Clinic, 1317 S. Hwy 32, El Dorado Springs, MO.

Documentation
In addition to completing a Financial Assistance Application, patients are also required to provide supporting documents to verify eligibility. Required documentation includes:
●Verification of all sources of income for past 3 months-- including paycheck stubs and all other forms of income of all members in the household
●Copy of the 3 most recent bank/investment/savings statement for all accounts of family members in household
●A complete copy of the most recent filed U.S. Individual Income Tax Return including all supporting schedules
●Medicaid denial letter

CCMH will not charge a financial assistance recipient more for healthcare services than the amount generally billed to individuals who receive the same care. An individual who qualifies for financial assistance may contact the hospital for information about the amount generally billed for their covered healthcare services.
