At Cedar County Memorial Hospital our Social Services Department is dedicated to supporting patients and families beyond their immediate medical needs. We understand that health and recovery can be influenced by many factors, including access to resources, transportation, finances, living arrangements, family support, and other challenges that may arise during or after a hospital stay.
Our Social Services Department works closely with patients, families, healthcare providers, and community partners to identify needs, coordinate services, and help patients navigate the resources available to them. Social Services may include:
●Care coordination and discharge planning●Connection to community resources and support services
●Assistance navigating Medicaid
●Transportation resources and assistance
●Coordination of home health, hospice, rehabilitation, skilled nursing, and other post-acute services
●Nursing home and long-term care referrals
●Durable medical equipment coordination
●Support with advance care planning and healthcare decision-making
●Assistance addressing social determinants of health
●Patient and family advocacy
