Social Services

At Cedar County Memorial Hospital our Social Services Department is dedicated to supporting patients and families beyond their immediate medical needs. We understand that health and recovery can be influenced by many factors, including access to resources, transportation, finances, living arrangements, family support, and other challenges that may arise during or after a hospital stay.

Our Social Services Department works closely with patients, families, healthcare providers, and community partners to identify needs, coordinate services, and help patients navigate the resources available to them. If you or a family member could benefit from Social Services, please speak with a member of your healthcare team or contact our Social Services Department.

Social Services may include:

Care coordination and discharge planning
